Manage Perio Forms & Information

The Perio EPR Forms window allows you to add and manage perio forms for the selected patient, and track perio information, such as current Perio Type, Prognosis, and Perio Screening and Recording (PSR) scores. The patient’s medical alerts, if any are on record, also display.

PSR scores are used as a diagnostic tool for the early detection of periodontal disease. Each box represents an intraoral sextant. Intraoral sextants are designated S1-S6 beginning in the maxillary upper right sextant (S1), proceeding in a clockwise manner, and finishing in the mandibular lower right sextant (S6). If your institution is set up to auto-calculate PSR scores, they are based on the patient’s most recent incomplete, complete, or closed exam and are calculated on the worst score within a sextant.

Example: If pocket depth is charted and there are pocket depths of 3.0 mm and 5.00 mm in the same sextant, the calculation will be based on the 5.0 mm depth.

Note: In the EHR module > Perio tab, the PSR scores are based on the selected exam.

If your institution is not set up to auto-calculate PSR scores, you will need to enter the PSR score values manually.

PSR scores indicate the following:

  • 0: The pocket depth is <3.5 mm and has no calculus or bleeding.
  • 1: The pocket depth is < 3.5 mm with bleeding.
  • 2: The pocket depth is < 3.5 mm with calculus or overhang.
  • 3: The pocket depth is >= 3.5 mm.
  • 4: The pocket depth is >= 5.5 mm.
  • *: If the checkbox is selected, a clinical abnormality is present in that sextant. Clinical abnormalities include but not limited to furcation invasion, mobility, mucogingival problems, or recession extending to the colored area of the probe (3.5+ mm).
  • X: An edentulous (missing) sextant.

To manage perio forms and information:

  1. Select a patient and open the Perio Chart module.
  2. Click the EHR Forms () icon to display the Perio EHR Forms window.

    3. Note: If the patient does not have any perio exams on file yet, the fields are completely blank.

  3. To add a perio form, complete the following.
    1. Click the Create New Form () icon to display the Add Form window.

    2. Complete the fields as necessary and click OK to add the form.
  4. To perform another task, complete any of the following:

    5. When necessary, you can edit saved, incomplete exams.

    Important: Once an exam is completed and approved by a faculty member or instructor, it cannot be edited.

    1. Select an entry from the Form drop-down.

      2. Note: If the entry is a multi-instance form with multiple instances on file, the Select Form window displays, prompting you to select the correct instance.

    2. Modify the form answers as necessary.

    Forms may be single- or multi-instance forms. If a multi-instance perio form is in use on the patient record, you may sometimes need to display a different instance of the form than is already selected.

    1. Click the Select Form () icon to display the Select Form window.

      2. Note: This icon does not display if a single instance form is selected

    2. Select a listed form instance and click OK.

    In extreme circumstances, it is possible to delete a perio form.

    Important: We do not recommend deleting perio forms.

    1. Select an entry from the Form drop-down.

      2. Note: If the entry is a multi-instance form with multiple instances on file, the Select Form window displays, prompting you to select the correct instance.

    2. Click the Delete Record () icon, and when prompted, click Yes to confirm the deletion.

    1. Select an entry from the Form drop-down and click the Show History icon () to display the Select Form History window.

    2. Select a listed date and click OK to display the View Form History window.

    3. Review as necessary.
    1. Select an entry from the Form drop-down.

      2. Note: If the entry is a multi-instance form with multiple instances on file, the Select Form window displays, prompting you to select the correct instance.

    2. Click the Add Action () icon to display the Add Form Action window.

    3. Complete the fields as necessary and click OK.
    1. Select an entry from the Form drop-down.

    2. Complete any of the following actions:

      • If the form is active, click the Suspend Form () icon and when prompted, enter a reason, and click OK.
      • If the form is suspended, select a form and click the Unsuspend Form () icon and when prompted, click Yes to confirm you want to resume the form.
    3. If required, get approval.
    1. Select a form and click the Print Report () icon to display the Print Form window.

    2. Select a radio button:

      • Standard Reports: This prints the form as it appears in the Perio Chart module.

        • Note: The standard Perio Form report uses the PtForm.rpt file.

      • Custom Reports: This uses a customized Crystal Report specific to the form.
    3. If you selected the Custom Report radio button, select an option from the drop-down list.
    4. (Optional) Click Preview to view the form.
    5. Click Print.

    You can view the patient’s current perio status, listed conditions, and current PSR scores at the top of the Perio EPR Forms window. When necessary, you can modify the information to keep it current with the patient’s progress.

    Note: Depending on your setup and institution’s requirements, axiUm may calculate the patient’s PSR scores automatically or you may need to enter scores manually.

    Each PSR score box represents an intraoral sextant. Intraoral sextants are designated S1-S6 beginning in the maxillary upper right sextant (S1), proceeding in a clockwise manner, and finishing in the mandibular lower right sextant (S6).

    1. Review the Perio Type and Prognosis fields, as well as the conditions text box, and the PSR scores.

    2. If necessary, modify the following fields:
      • Perio Type: Select the patient’s perio classification.
      • Prognosis: Select the patient’s expected development of the condition and likely course for treatment.
      • Patient ASA: Select the ASA level that applies to the patient.

        • Note: This field only displays if the form is configured to trigger ASA values at the page level.

      • PSR: If enabled, enter the correct value for each sextant.

        • Note: Any changes are saved automatically.

    You can review past perio statuses and PSR scores.

    1. In the text field, right-click and select Show Perio Status History… to display the Show Perio Status History window.

    2. Select a listed entry to view the perio type, prognosis, and PSR scores.

    If the patient has any medical alerts on their record, they display in the available text box.

    Note: Medical alerts only display if the EHR Display Options window > Medical Alerts checkbox is selected during setup.

    You can quickly access the patient record from within the Perio Chart module.

    1. Click the Patient Card () icon to display the Patient Card window.

    2. Review the information as necessary.
